A step in the cleaning process of surgical equipment was missed at Goshen Hospital in Indiana, possibly exposing about 1,182 surgical patients to HIV, hepatitis C, and hepatitis B between April and September 2019. Patients were notified out of extreme caution though the risk for exposure was very low.
